Check Box Added

On the application form (reports> patron application> application form there is a checkbox currently at the bottom that says EZMEAL APP Only.

The enhancement I am asking is to have a checkbox for Reports>Patron Applicaition> Income form where it says EZMeal APP Income Form ONLY
